Yes, I remember trying to answer their questions. Not worth the $10. From memory, it was like:
1) When, where and from whom did you buy the computer?
2) How much did you pay for the computer?
3) Etc., Etc, Etc.

The web site says "The cheques for the approved consumer payments will be sent by the end of summer 2022 via regular ground mail."There is still over two months before summer 2022 ends around September 22.
"You can expect to receive payment in the late spring or early summer of 2022."
This is what the payment approved email said back in May...
Confirmed I totally forgot about it and this thread reminded me 🙂

Update as of August 23, 2022 … The cheques for the approved consumer payments have been sent via regular ground mail.
