Not sure why you need to insist on paper statements. You can get monthly PDF versions from most banks that you can easily download. Even if they do not offer that, you can always regularly (monthly?) "print" a copy of online transactions to a PDF file to store on your computer, though this is required for very few banks.

When I submitted a direct transfer authorization form to link another FI, although the form indicates a mailing address, I called the CSR who gave me the following fax number 877 728 5822.

The explanation was that they initially require snail mail for incoming documentation from new clients, but "established" clients may use fax.

Is the cheque scanned as an attachment to the Direct Transfer Authorization form and both was faxed in?

There is a Direct Transfer Authorization form you fill out and fax. I have done it a while ago. I must have gotten the form on the site somewhere. Note sure where I got the fax # but it is 1-877-728-5822 (I think I got it from them via phone).

Yes, two separate PDF documents. Check with VOID written across the front.

The Direct Transfer Authorization form is downloaded as a PDF once you are logged into your account, and select Account Services | Direct Transfer Authorization.

The form download link appears to give you the form even if you are not logged in.
